Success Story Reuniting Rick and Mary

Bringing Loved Ones Back Together Where They Belong

At Liberty Home Care, we believe that home is more than a place—it is where people belong with those they love.

When Rick and Mary first became our clients, they had been married for more than forty years and had spent most of their lives together in the same home. Both had been diagnosed with dementia, but their journey took an unexpected turn when Mary’s condition worsened. After experiencing significant behavioral changes, she was admitted to a nursing facility’s memory care unit for her safety.

The Challenge of Living Apart

Rick remained at home, living alone for the first time in decades. Although he also had dementia, his greatest struggle was the loneliness of being separated from his wife. Mary, meanwhile, was equally unhappy in the facility despite receiving the care she needed.

From the beginning, our mission was clear: to reunite them safely at home.

Our Care Plan

We provided Rick with a dedicated daily caregiver who assisted with his personal care, meal preparation, housekeeping, medication reminders, and companionship.

For Mary, we arranged twice-weekly caregiver visits that allowed her to leave the memory care unit for fresh air, walks, community outings, and meaningful recreation, helping her maintain a connection to the outside world.

For more than six months, our team worked closely with their physicians, specialists, therapists, and family members. We attended appointments, coordinated care, monitored their progress, and helped develop a plan that would allow Mary to safely transition back home.

Bringing Mary Home

After months of collaboration, preparation, and persistence, Mary was finally discharged from the memory care facility and reunited with Rick in the home they had shared for so many years.

Today, Liberty Home Care continues to support both Rick and Mary so they can remain together despite the challenges of progressive dementia

How We Continue to Help

Nancy's Final Wish: Home Until the Very End

“Honoring a life lived with dignity, compassion, and the comfort of home.”

Some people leave behind memories that become part of who we are. Nancy was one of those remarkable people.

Nancy was 101 years old when she peacefully passed away while receiving care from Liberty Home Care. Her greatest wish was simple yet deeply meaningful—to remain in the home she had loved for more than 70 years until her very last breath.

With no close family living nearby, Nancy relied on Liberty Home Care not only for professional caregiving but also for companionship, kindness, and the reassurance that someone would always be there. It was both an honor and a privilege to help make her final wish a reality.

Adapting Care Every Step of the Way

As Nancy’s needs evolved, so did the care we provided. Our team gradually increased support as her health declined, eventually providing 24-hour care so she would never be alone.

When it became clear that end-of-life care was needed, we coordinated hospice services directly in her home. This allowed Nancy to remain in the familiar surroundings that brought her the greatest comfort and peace.

Caring Beyond Daily Needs

Our commitment extended far beyond personal care.

We prepared Nancy’s favorite meals, coordinated her medical appointments, and paid close attention to the small details that made each day meaningful.

Knowing how much she loved flowers, we planted her favorite blooms outside her bedroom window. We even rearranged her room so she could comfortably look outside each day and enjoy the colorful garden she cherished.

These simple acts helped preserve the joy and familiarity that made her house feel like home.

More Than Caregivers — Family

Nancy loved sharing stories from her remarkable century of life.

She spoke about the world she had witnessed changing, the people she had loved, and the memories she treasured most. Our caregivers looked forward to every conversation. They laughed together, reflected together, and formed a bond that became much more than caregiver and client.

Over time, Nancy became part of the Liberty Home Care family.

“Home is where comfort, dignity, and love remain until the very end.”

Honoring Her Final Wish

One quiet evening, Nancy passed away peacefully in her own bed—exactly where she had always hoped she would be.

One of our caregivers remained by her side, gently holding her hand as she took her final breath.

Following Nancy’s previously expressed wishes, our caregiver calmly contacted the hospice nurse and funeral home, ensuring every step was handled with dignity, compassion, and respect.

A Legacy That Lives On

Losing Nancy was heartbreaking for our entire team.

We miss her warm smile, her stories, and the joy she brought into our lives. Yet alongside our sadness is a deep sense of peace, knowing we fulfilled the promise we made to her.

Nancy remained in the home she loved until the very end, surrounded by compassionate care, comfort, and respect. Her wishes were honored, and her life was celebrated with the dignity every person deserves.

At Liberty Home Care, we believe home is more than a place—it is where memories live, where love remains, and where life’s final chapter can be written with comfort, compassion, and dignity whenever possible.

Nancy’s story reminds us why we do what we do every single day.

❤️ In Loving Memory of Nancy

Thank you, Nancy, for allowing us to be part of your remarkable journey. Your life, your strength, and your spirit will forever remain in our hearts.

A Sister's Greatest Gift - Liberty Home Care

Sometimes the Greatest Heroes Are Family

Some heroes don’t wear uniforms. They don’t seek recognition or applause. They simply show up every day with love, compassion, and an unwavering commitment to the people who need them most.

For Hanna, caring for others was nothing new.

She had built a successful career in medical research, dedicating years of hard work to advancing healthcare through science and innovation. It was a career she was proud of—one that offered purpose, stability, and the opportunity to make a meaningful impact.

Then life changed.

Her sister, Linda, and brother-in-law, Adam, were both diagnosed with multiple serious illnesses. Dementia gradually stole cherished memories, while cancer, heart failure, and other chronic conditions made everyday tasks increasingly difficult. As their health declined, it became clear they could no longer live safely or independently on their own.

Without hesitation, Hanna made an extraordinary decision.

She stepped away from the career she had worked so hard to build and became their full-time caregiver.

It was not an easy choice.

Leaving behind professional success, financial security, and a career she loved required immense courage. Yet for Hanna, the decision was simple. Family came first.

Every day became an act of love — helping with daily routines, managing medical appointments, providing comfort during difficult moments, and ensuring Linda and Adam could continue living with dignity and compassion.

Eventually, Hanna came to Liberty Home Care.

She wasn’t looking for sympathy.

She was looking for an opportunity to continue caring for her loved ones while earning enough to support herself. During the hiring process, we immediately recognized something remarkable. Beyond her professional qualifications, Hanna possessed the qualities that define an exceptional caregiver: genuine compassion, patience, resilience, and an unwavering dedication to the people she loves.

We were honored to welcome Hanna to the Liberty Home Care family.

Understanding her unique circumstances, we worked with her to provide the flexibility and support she needed so she could continue caring for Linda and Adam while building a meaningful career with our team.

Today, Hanna often describes her paycheck as “a little money.”

But what she has gained is far more valuable.

She has the peace of mind of knowing that the people she loves most receive compassionate, professional care every single day. She has been able to preserve their dignity, improve their quality of life, and remain present for both the difficult days and the precious moments that matter most.

Her journey reminds us that caregiving is about far more than completing daily tasks.

It is about preserving independence.

It is about honoring dignity.

It is about bringing comfort during life’s most challenging seasons.

Most importantly, it is about love.

At Liberty Home Care, we believe every caregiver has a story, and every story reflects the extraordinary compassion that transforms lives. We are proud to support dedicated caregivers like Hanna, whose selflessness reminds us that the greatest acts of kindness often happen quietly, behind the scenes, within the people we call family.

Their compassion inspires us every day, and their commitment reflects the heart of everything we do.
