When Rick and Mary first became our clients, they had been married for more than forty years and had spent most of their lives together in the same home. Both had been diagnosed with dementia, but their journey took an unexpected turn when Mary’s condition worsened. After experiencing significant behavioral changes, she was admitted to a nursing facility’s memory care unit for her safety.
The Challenge of Living Apart
Rick remained at home, living alone for the first time in decades. Although he also had dementia, his greatest struggle was the loneliness of being separated from his wife. Mary, meanwhile, was equally unhappy in the facility despite receiving the care she needed.
From the beginning, our mission was clear: to reunite them safely at home.
Our Care Plan
We provided Rick with a dedicated daily caregiver who assisted with his personal care, meal preparation, housekeeping, medication reminders, and companionship.
For Mary, we arranged twice-weekly caregiver visits that allowed her to leave the memory care unit for fresh air, walks, community outings, and meaningful recreation, helping her maintain a connection to the outside world.
For more than six months, our team worked closely with their physicians, specialists, therapists, and family members. We attended appointments, coordinated care, monitored their progress, and helped develop a plan that would allow Mary to safely transition back home.
Bringing Mary Home
After months of collaboration, preparation, and persistence, Mary was finally discharged from the memory care facility and reunited with Rick in the home they had shared for so many years.
Today, Liberty Home Care continues to support both Rick and Mary so they can remain together despite the challenges of progressive dementia
